Medical Enrollment

Eligibility

Eligible members must reside in the state of Oregon, and/or reside in specific service areas of the plan.

Underwriting

Applicants must be approved by ODS underwriting using the Oregon Standardized Health questionnaire. Effective dates are assigned by underwriting and are generally the first day of the month following underwriting approval. Agents are required to submit a clean and complete application by the 20th of the month for the next month's effective date on the 1st. If a 15th effective date is requested, we must have your application by the 5th day of the requested month. Applications received after the cut-off date will be processed for the subsequent month's effective date, beginning the 1st or 15th as indicated, unless "first available" is selected.

Billing

Members have the option to pay monthly by electronic checking account deduction, monthly via a billing statement, or quarterly via a billing statement. Premium must be collected and sent in with the application.

Dental Enrollment

Eligibility

Eligible members must reside in the state of Oregon and be enrolled in an Individual Medical Plan.

Members must choose the dental plan upon original enrollment in one of the individual medical plans (see underwriting for additional dental plan eligibility rules).

Underwriting

Applicants must be approved by ODS underwriting. Effective dates are assigned by underwriting and are generally the first day of the month following underwriting approval.

Dental plan members must choose the dental plan upon originally enrolling, otherwise will not be able to select the dental plan at a later date. If a member drops the dental plan, it cannot be reinstated. If a member drops an individual medical plan, they cannot retain the dental plan.
